changes as far as I know.

that I had made up in my head,

because that's how he made me feel
every time I talked to him about it.

because it wasn't true.

This just happened.

[newsman] …remnants of an ill*gal
after-hours club this morning.

Apparently, about 5:00 a.m., a
pyrotechnics misfire on stage

engulfed the building in flames
and caused a stampede for the exit.

We hear there are no fatalities,
but a lot of injuries.

- Oh sh*t.
- [Luis] Mm-hmm.

- [Elijah] Where is that?
- Fifteen minutes away.

- They're all coming here?
- Oh yeah.

- [heartbeat thumping]

In exactly seven hours and 14 minutes,
my intern year, AKA hell on earth,

is over.

- How you gonna celebrate?

I'll just probably have a
drink at the party tomorrow

and surgery with Soriano today.

- Are you joking?
- No.

- He asked me to scrub in on Nia's surgery.
- And what about Cole?

[chuckles wryly]

He is so pissed
at him right now.

And honestly, it is making
my day so much better.

- Soriano's pissed at Cole?
- Yeah.

Why? What happened?

[sighs] He went
to some boat party

with Dr. Hoffman and a bunch
of his plastic surgery bros.

What, he didn't like that, or…

move politically.

I'm pretty sure he's
involved with Nia.

[wailing sirens approaching]

[music intensifies]

- How'd things go with Cruz?
- She wasn't thrilled.

Sounded like that guy was being
a d*ck. I'm sure she understands.

[EMT] Coming through!

- [Phillips] What we got?
- 22-year-old trying to leave the club.

Broke a glass window, leapt
from the second story.

Has difficulty breathing with large glass
lacerations along his arms and his legs.

- Vitals are stable with a GCS of 14.
- All right. Trauma one's open.

[Danny] Okay, on
three. One, two, three.

- What's your name, sir?
- Garrett Donnelly.

All right, man. We're gonna
take good care of you, okay?

Whoa. Deep laceration
to the upper right arm.

- Can I get a stapler?
- [Luis] Yes.

- [Garrett] That was nuts, man.
- [Phillips] Was it?

All right. This is just gonna
feel like a few pinches, okay?

[Garrett] Okay.

[stapler clicking]

- How are you feeling, Mr. Donnelly?
- Oh, I feel fine.

Yeah? Are you on
anything right now?

- [chuckling] You mean like drugs?
- Yeah, like drugs.

Yeah, I'm on G and some Tusi.

[Phillips] Hmm?

Tusi is a mix of
a lot of things.

MDMA, ketamine,
psychedelics, cocaine…

[music recedes]

- Fun.

It's your last day of your
year of rotation, isn't it?

- Mm-hmm.
- Alright. Gown up.

about running a trauma.

the primary survey, ABCs.

airway's intact.

And then bladder.

Dr. Phillips. This is Dr. Simms.

- Can you tell us what happened?
- Yeah, bro.

People were stampeding
everywhere, so I went upstairs,

and then there was so much
smoke and nowhere to go.

I jumped out the window.

Feels like a closed femur fracture on
the right, and also maybe the left.

We should get him to CT.

I was falling, and, you
know, it was so far,

and then I was like, "Oh,
sh*t, that's gonna hurt."

- And it did.
- Yeah, I bet it did.

Camila, anything else we need
to do before sending him to CT?

Um…

Any suggestions for
our student here?

No, she's doing great.

Exposure. Make sure you check
everything on the patient.

Yeah, I did. We did.

[Phillips] You sure?

Oh! I forgot to roll him.

[EMT] Multiple crush injuries, first and
second-degree burns. Vitals are stable.

[Harper] Hey, can you
tell us your name?

Penny Navarro, are
my friends here?

- Do you guys need help?
- Uh, sure. Let's see what's going on.

Hey, Penny. How are you feeling?

What hurts?

My skin. It burns. My
legs. It's hard to breathe.

Lungs clear bilaterally.

I lost my friends, Charlie
Ramirez and Fiona Arnez.

- Are they here?
- You're one of the first patients.

Abdomen's soft.

[Penny] We were having one last night
out before summer and college, and…

then the fire started,
and the crowd went crazy.

I got knocked down,

[Danny exclaims]

They're fake, but the

- FAST is negative.

[indistinct PA announcement]

[door opens]

Dr. Soriano.

- What can I do for you, Dr. Cole?
- Um…

I just wanted to let you know before
you scrub in on Nia Washington

that I had GI radiology
go back over her CT scans

to check the intra-abdominal
abscess wasn't a problem

given its proximity to the IVC.

[Soriano] Mm-hmm…

With the surrounding
inflammation,

they said they can't say for sure that
it's not eroded into the outer wall.

[Soriano] Mm-hmm…

Is that all?

Well, actually, I just
wanted to say, sir,

it's an honor to work with
you and learn from you,

and I don't want to do
anything to jeopardize that.

I'm sorry that I
wasn't here yesterday.

But Nia's my patient, and I'd appreciate
the opportunity to scrub in today.

[Cole] Uh…

We were just discussing
the nightclub fire.

There are a lot of
traumas coming in.

Dr. Chan thought she might be
more useful down in the ER.

from some busted ribs.

It's only about 15%
of his lung volume.

He's an otherwise healthy guy,

so I don't think he needs a chest tube,
but I just wanted to get your opinion.

Do I think you should do a
painful invasive procedure

on a pneumo that'll
likely heal on its own?

No.

- Is that all?
- Yeah.

Yeah, thanks.

Was trampled at the club fire.

She had no LOC.

Patient has facial trauma,
she has some contusions,

she also has sporadic burning as
well as a dislocated shoulder.

Let's get her moved, folks,
get her clear. On my count.

[all grunt]

Our patient's been speaking
with slurred words,

is also asking
questions repeatedly.

From suspected substance use.

All right, let's go.

Hey, Fiona. My name is Dr. Sam
Elijah. How are you doing?

[slurring] Just great. Peachy.

What'd you take tonight?

[sighs]

It's okay, you're not
gonna get in trouble.

We all took some MDMA.
Me and my girlfriends.

Do you have any other medical
problems? Are you on any medications?

Just myocycline for acne.

[Elijah] Let's send out trauma
labs, get her a liter of NS.

Fiona, I'm Dr. Simms, and I wanted
to let you know that Penny is here.

She is. She has a few injuries,
but she's going to be just fine.

right there, and we'll drain the pus.

- [suction hissing]
- [classical music playing]

[Soriano] That's good.

Wow, that's a lot of pus.

Oh sh*t, it eroded right through the
IVC. Suction, we've got to get control.

Clamps? Here, put your
finger in right there.

[Cole] Looks like there's a small
hole that the abscess adhered to.

- [Soriano] Need to control venous inflow.
- [Cole] Vascular clamps.

[Soriano] No, the IVC's extremely friable.
That's going to tear it open even more.

Give me a sponge stick.

Now move your
finger. One, two, go.

- Suction.
- [Cole] Mm-hmm.

[equipment beeping and chiming]

preventing the flow of blood

to your optic nerve and retina, and
we have to relieve the pressure.

She's gonna do something
called a lateral canthotomy.

It'll relieve the pressure behind
the eye and save your vision.

- Penny.

[Fiona] Thank you.

How is she, Doctor?

Well, she's out of surgery.

Yeah, she's in critical condition,
but she's stable at the moment.

She had a deep infection
in her abdomen.

She had some issues with bleeding
that we were able to control.

Is she going to be okay?

[Soriano] We're hopeful
that she will be.

We're doing everything
we can for her.

How soon will you know
if she's gonna be okay?

We're keeping her
closely monitored.

We really should know a lot more
in the next day or two. Okay?

In the meantime... I'm sorry.

This is Dr. Cole. He's
my surgical resident.

And Dr. Cole is gonna be running
point on Nia's post-surgical care.

- Tom Cole?
- [Soriano] Yes.
